RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT WITH PARENTAL CONSENT (“AGREEMENT”)
IN CONSIDERATION of being permitted to participate in any way in any wrestling event (“Activity”) at any time during the current calendar year I, for myself, my personal representatives, assigns, heirs, and next of kin:
1. ACKNOWLEDGE, agree, and represent that I understand the nature of the Activity and that I am qualified, in good health, and in proper physical condition to participate in such Activity. I further agree and warrant that if, at any time, I believe the conditions to be unsafe, I will immediately discontinue further participation in the Activity.
2. FULLY UNDERSTAND that: (a) THIS ACTIVITY INVOLVES RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, AND DEATH (“Risks”); (b) these Risks and dangers may be caused by my own actions or inactions, the actions or inactions of others participating in the Activity, the conditions in which the Activity takes place, or THE NEGLIGENCE OF THE “RELEASEES” NAMED BELOW; (c) there may be OTHER RISKS or SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation, or that of the minor, in the Activity.
3. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE the sanctioning organization(s), their administrators, directors, agents, officers, members, volunteers, and employees, other participants, officials, rescue personnel, sponsors, advertisers, owners and lessees of Premises on which the Activity is conducted, (each of the forgoing shall be considered one of the RELEASEES herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED, OR ALLEGED TO BE CAUSED, IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS; AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my behalf, makes a claim against any of the Releasees, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may be incurred as the result of such claim.
I ACKNOWLEDGE THAT I AM AGE 18 OR OLDER, HAVE READ THIS AGREEMENT AND FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, HAVE SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE, AND I INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF
ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID, THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT.
MINOR RELEASE: (must be completed by Parent/Guardian for any participant under the age of 18) AND I, THE MINOR’S PARENT AND/OR LEGAL GUARDIAN, UNDERSTAND THE NATURE OF THE ACTIVITY AND THE MINOR’S EXPERIENCE AND CAPABILITIES AND BELIEVE THE MINOR TO BE QUALIFIED, IN GOOD HEALTH, AND IN PROPER PHYSICAL CONDITION TO PARTICIPATE IN SUCH ACTIVITY. I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS EACH OF THE RELEASEES FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON THE MINOR’S ACCOUNT CAUSED, OR ALLEGED TO BE CAUSED, IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE “RELEASEES” OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS AND FURTHER AGREE THAT IF, DESPITE THIS RELEASE, I, THE MINOR, OR ANYONE ON THE MINOR’S BEHALF MAKES A CLAIM AGAINST ANY OF THE RELEASEES NAMED ABOVE, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS LIABILITY, DAMAGE, OR ANY COST THAT MAY OCCUR AS A RESULT OF ANY SUCH CLAIM.
Media Consent & Release
I hereby grant Golden Eagle Wrestling Club and their agents the absolute right and permission to use photographic portraits, pictures, digital images or videotapes of me or my child, or in which I or my child may be included in whole or part, or reproductions thereof in color or otherwise for any lawful purpose whatsoever, including but not limited to use in any publication and team’s websites, without payment or any other consideration. I hereby waive any right that I may have to inspect and/or approve the finished product or the copy that may be used in connection therewith, wherein my likeness appears, or the use to which it may be applied. I hereby release, discharge, and agree to indemnify and hold harmless Golden Eagle Wrestling Club and their agents from all claims, demands, and causes of action that I have or may have by reason of this authorization or use of my or my child’s photographic portraits, pictures, digital images or videotapes, including any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said images or videotapes, or in processing tending towards the completion of the finished product, including publication on the internet, in brochures, or any other advertisements or promotional materials.
Youth Wrestling Parent/Coach Code of Conduct
Children’s sports are supposed to be fun – for children. Unfortunately, many parents, fans, and coaches don’t realize that
their actions, whether verbal or nonverbal, can have a lasting emotional effect on children. Too many children are leaving
sports activities because the fun is unfairly taken away by adults. Parents, fans, and coaches who follow this simple code can
help reinforce what sports are all about…
BEING FUN FOR EVERYONE.
Preamble The essential elements of character-building and ethics in sports are embodied in the concept of sportsmanship and
six core principles: trustworthiness, respect, responsibility, fairness, caring, and good citizenship. The highest potential of
sports is achieved when competition reflects these “six pillars of character.” As a parent or spectator:

I will inform the coaching staff of any physical and or mental conditions that may affect the safety of my child and others.

I will review and abide by the Golden Eagle Wrestling Clubs policies and expectations at practices and in competitions.

I, my wrestler, and my guests will be positive role models by encouraging sportsmanship, being respectful, and demonstrating positive support for all wrestlers, coaches, officials, and spectators at practice and in competitions.

I, my wrestler, and my guests will not engage in unsportsmanlike conduct with teammates, officials, coaches, opposing wrestlers, parents, or spectators at practice and in competitions.

I will promote the emotional and physical well-being of all athletes, coaches, parents, and spectators at practice and in competitions. I will not encourage behavior that may endanger the health and or well-being of my wrestler, fellow teammates, opposing wrestlers, parents, or spectators at practice or in competitions.

I will respect the coaches and officials’ authority at practice and in competitions. If an issue arises at a competition, I will let our coaching staff know and let them handle it. If I have a question or concern regarding my child, the club, or the coaching staff I will contact the head coach directly to set up a meeting to discuss the matter at the appropriate time and in the appropriate place.

I will refrain from coaching my child and other athletes at practices and in competitions unless I am an official parent volunteer. All parent volunteers must be approved by the head coach and complete the certified coaches training that includes, a background check, safe sport training, and a valid USA Wrestling Leader membership.

I will ensure my child wears designated Golden Eagle Wrestling Club attire at practice and in competitions unless previously approved by the Club.
Golden Eagle Wrestling Club has a ZERO TOLERANCE policy regarding un-sportsmanlike and bullying behavior. At any time if a wrestler or parent violates these conditions a verbal and written reprimand will be provided along with appropriate disciplinary actions to include possible suspension from practices or competitions and/or termination from our wrestling program. If a wrestler is terminated from the wrestling program then there will be no refund.
